Cityblock partners with Sunshine Health to provide value-based primary care services in Central Florida
Cityblock, a Medicaid value-based primary care company, has joined forces with Sunshine Health, a Centene subsidiary, to provide essential care services to high-need Medicaid members in 11 Central Florida counties. This partnership, which kicked off this month, aims to offer comprehensive care coordination services to members, including 24/7 support across various modalities such as virtual visits, home visits, and clinic appointments.
Cityblock’s unique care model is specifically tailored for individuals with complex needs who are at a higher risk for poor health outcomes. By providing a multidisciplinary care team, the company hopes to reduce unnecessary emergency department visits and hospital admissions while enhancing the quality of care and member engagement. Their 2024 Equity in Action Report has already shown promising results, with increased engagement, clinical continuity, and significant reductions in avoidable inpatient admissions among these populations.
The partnership will initially serve members in counties such as Volusia, Seminole, Orange, and Hillsborough, among others. Dr. Toyin Ajayi, co-founder and CEO of Cityblock, expressed excitement about expanding their operations into Florida and strengthening their national partnership with Centene through this collaboration with Sunshine Health.
This partnership is part of a larger trend for Cityblock, as they recently announced a similar partnership with Senior Whole Health by Molina in Massachusetts. With a focus on dual eligible Molina members, the company is dedicated to providing personalized medical, behavioral health, and social care support to enhance the overall well-being of these individuals.
Centene’s financial success in 2023, with $2.7 billion in profit and $154 billion in revenue, reflects the growing demand for innovative healthcare solutions like those offered by Cityblock and its partners. This collaboration highlights the importance of addressing the unique needs of Medicaid members and improving access to quality care services in underserved communities.
